Dear Friends of Hospice,
We are announcing the fifth annual fundraiser for our Angels Anonymous Golf Tournament. Angels Anonymous is an organization exempt from income tax according to internal revenue code SECTION 501(c)(3) supported by Horizon Home Health & Hospice, Inc. Angels Anonymous funds are used to provide services and personal items to Hospice patients who are in need, but could not otherwise afford to purchase them. Thanks to the golf tournaments, your generous donations, and the loving memorials offered by families and friends, we are able to improve the quality of life for many patients and families.
If you wish to support this heartfelt effort, you may do so in the following ways:
1. You and your golfing pals can sign up for the tournament.
2. You or your business can sponsor a hole. (Your logo will be displayed on a provided sign at a tee box. You may have a table or display if you desire.)
3. Whether or not you golf, we would love to see you afterwards at the BBQ. Tickets are $20 each and must be purchased in advance as we need a headcount for the caterer.
4. You may donate an item for the silent auction or raffle.
5. You may purchase raffle tickets in advance for $1 each or 6 for $5.
6. Any cash donation would gladly be accepted as well.
We truly appreciate any support you give to help us carryout our mission which states, "...whenever possible, Horizon Hospice will not turn away a qualified patient for inability to pay, and will remain committed to the provision of quality care to all patients who are terminally ill."
